Insurance and Billing Information

You are responsible to pay co-payments on the date of service and patients are expected to pay balances.

We welcome most insurance plans. A non-comprehensive list of insurance plans we accept is: AARP, AETNA, BCBS, CHHP Humana National EPO, HMO, MISC, CHHP Unicare PPO, Cigna, HFN, Medicare, Monumental Life Insurance, Mutual of Omaha, PHCS, PHN, Prudential, Travelers Medicare Railroad and United Health Care.  To insure that we accept your plan, please contact our billing department or your insurance company directly.

You will need your insurance card at each office visit. In order to file claims to your insurance company, we must have correct and updated information.
